Plot:

John Rambo (Stallone) spends his time rebuilding villages and helping poor people in the daytime, while fighting UFC style in the nights for some extra bucks, still not keen to live life back in the 'real world' due to his violent past and distrust of governments. His old Army Colonel and only friend Trautman tracks him down to ask for his help as they help the Afghan people rise up against the Russians ... but Rambo refuses to sign-up.
    While the colonel is on a mission in Afghanistan he is taken captive by a nasty Russian general, with a thing for helicopter gunships, and tortured.  Looks like it's up to Rambo ... again ... to save the day.

 
So, how does it end?

Going it alone, Rambo returns to the Soviet base and breaks out the Colonel and several other prisoners, just before Trautman is about to be toasted with a blow-torch.  They flee the base in a gunship, which crashlands in the desert.  Salvaging a small arsenal of weapons, they hide out in a cave - but are soon tracked down by Russians soldiers.  All John needs is his trusty bow, and he takes those poor Ruski's out one by one -- including a very imaginative sequence for the biggest dude right at the end.
    Rambo and Trautman trudge along through the desert and think they hear a noise.  They turn around and, oh yes, a slow moving Russian army full of tanks and old-school jeeps has crept up on them.  Rambo takes out a few jeeps with a grenade launcher, and then the Rebels show up on horseback and cause chaos.  Basically all the Russians are soon shot and/or blown up.  Rambo is still shirtless.

  The Russian chopper is still hanging around, with the bad general hoping to take out the big man.  Rambo is finally shot in the leg.  He finds a molotov cocktail and launches it at a tank, then gets inside and takes it for a joy ride.
  Building up the tension he surges forward, misses the chopper a few times for effect, then machine gun fires the general-pilot, and then slams the turret straight through the front windshield of the helicopter and the bad guy's skull.

The rebels are happy.  The little boy asks John if he'll stick around.  He says no.

 
Notes
It all ends in the final credits with a completely random choice of song not in any way keeping with the rest of the movie. You've got to check it out.
