Plot:

Jake Molloy (Stallone) is a Fed working the case of a psychopath killing cops in particularly gruesome ways.  Without any leads or hunches, suddenly the case becomes personal when a drinking-buddy is murdered ... and while at the crime scene the killer calls Molloy to tell him his plan - to kill his girlfriend.
  Unable to save those around him, Molloy chases the killer through a warehouse and corners the man - who first takes his own life.  His life spiralling into depression and alcoholism, his commanding officer books him into a cop-only rehab center in the frozen north.  However it is not long before the killings start again and as a storm closes in, it seems no one will make it out alive...

 
So, how does it end?

Everyone is a suspect until they die - especially Tom Berenger and Robert Patrick as the 'bigger name' actors, but Berenger is wasted in his role here.
  Molloy's Fed captain finds a dead body in an ice-lake while out fishing, and the local shop keeper confirms it as one of the cop patients on his way to the army-base.  Stallone works this out at the same time and while in the morgue he (for some unknown reason and 100% working on some kind of weird hunch here) lifts the eyelids of the first murdered patient to reveal the weakest of all serial-killer-in-a-movie taunts of "I C U".
  The inmates turn on Stallone, but he escapes through the air-con vents and goes to help them anyway.
  Slater, the English patient (why are the bad guys always English?) turns out to be the psychopath and electrocutes the old guy, ice-picks some others and shoots the rest until the final fight with Stallone, the nurse and the captain.
  In no time the bad guy is picked up and thrown onto a combine-harvester's lethal metal rack ... twice.  Just to make doubly-sure this time.
  Stallone now has his mojo back and can live to save the world another day.
